Been dealing with an issue very similar to issue 0005383.

Hardware: Supermicro X9SCL-F mobo, E2-1230v2, 4x2TB WD HDDs, LSI 9266-4i. Disks are in a RAID 10 array.

In it's most recent occurrence, I only need to install Centos 6.5 (2.6.32-431.11.2.el6) and within a few hours (or even less), I'll get IO errors, rendering the machine unusable:
http://robertclarke.com/s/Uz0.png [^]

Have seen this issue show up multiple times on both 9266-4i and 9271-4i cards (RAID 10 arrays) with both SSD and HDD configs. Has been occurring since December of last year, don't have exact kernel that it started occurring.

Have talked with a few others with the same cards having the same issue, see also: https://vpsboard.com/topic/3849-lsisupermicro-problems/?hl=%2Blsi#entry57042 [^]
