This is a Dell PowerEdge R310 with DRAC 6.

Kernel: 2.6.32-220.7.1.el6.ehci_module.x86_64

Problem manifests in _all_ centos 2.6.32-220-* builds.
Problem also exist on Ubuntu 10.04 LTS where kernel is similar (Identical in the ehci_endpoint_disable function I believe).

Performing a racadm racreset makes the DRAC reboot and also it's internal virtual USB connections disconnect. This results in a hang where a kernel thread hangs in ehci_endpoint_disable, and eventually the entire system hangs.

I can trigger this 10 times out of 10 on all of our R310 models so far.

I will try to run a debug kernel and see what gives too. There's probably a patch for this in a newer upstream kernel somewhere, but I haven't found the correct one yet.
